ЛІКУВАЛЬНА ТАКТИКА ГОСТРИХ ЕПІГЛОТИТІВ У ДОРОСЛИХ

Abstract
Acute epiglottitis (AE) is a severe bacterial disease of the laryngopharynx, causing inflammation of the epiglottis and also quite often can affect true and false vocal folds, laryngeal ventricles, cricothyroid cartilage and surrounding tissues, has a rapid course and with untimely diagnosis or inadequate treatment leads to serious complications (phlegmon of the neck, mediastinitis) and even patient's death. In this study we have evaluated 86 patients with AE aged from 20 to 81 who were treated according to the type of the disease (infiltrative or abscess), ultrasound diagnosis of microbiological landscape, and the results of biochemical investigation. According to the results of our study, the following conclusions are drawn: the approach of the treatment depends on the form of the disease: infiltrative or abscess. At first, the traditional anti-bacterial therapy ex juvantibus, traditional anti-inflammatory, detoxification and anti-fibrinolytic therapies are administered to the patient with acute epiglottitis. Conservative therapy should also include ascorbic acid in order to improve reparative processes in the cartilage of the epiglottis. Besides, patients need long-term prescription of NSAIDs and hepatoprotectors as biochemical parameters in acute phase do not become referential even one month after the disease.
