Long term effects of different physical activity levels on cardiovascular risk: results of a prospective 17-year follow-up study of middle-aged men

Abstract
To analyze longitudinal effects of different levels of leisure time physical activity (LTPA) on cardiovascular diseases (CVD) risk. 329 healthy men aged 41.3 ± 7.1 were prospectively observed for CVD in 1980-2005 (17.6 years). Body mass index, waist circumference, blood pressure, plasma glucose and lipids and 10-year risk of fatal CVD (SCORE) were measured. LTPA groups were categorized according to tertiles of mean energy expenditure (EE).The odds ratios (OR) and 95% confidence intervals (CI) of obesity, central obesity, hypercholesterolemia, hypertriglyceridemia, hypertension, low HDL-cholesterol level, elevated glucose and metabolic syndrome in the group with EE > 1975 MET/min/week were: 0.44 (0.21-0.93, p<0.001); 0.36 (0.14-0.59, p<0.001), 0.53 (0.33-0.99, p<0.05); 0.35 (0.11-0.55, p<0.001); 0.53 (0.31-0.99, p<0.05); 0.32 (0.15-0.69, p<0.01); 0.48 (0.24-0.55, p<0.01); and 0.30 (0.12-0.62, p<0.001), respectively, as compared to a group with EE < 750 MET/min/week. The OR of high 10-year CVD risk was significantly lower in the most active group as compared to the group with EE <750 MET/min/week (OR=0.54; 0.35-1.31, p<0.05). EE of 750-1975 MET/min/week was associated with lower CVD risk only in crude analysis. There is a significant inverse relationship between long term LTPA and CVD risk. High EE is a strong protective factor in development of cardiovascular risk. Insufficient influence of moderate LTPA level on CVD risk should be an issue of further research.
