Management of Airway and ventilation during tracheal stenting procedure

Abstract

Context: Management of Airway and ventilation for a surgery involving the airway is a challenge to the anaesthesiologists. Tracheal dilatation and stenting is a new procedure for treatment of subglottic stenosis. A 31 years old male, presented with stridor following prolonged intubation and ventilation, after organophosphorus compound poisoning 5 months back. He was diagnosed as a case of subglottic stenosis. Emergency tracheostomy was performed to relieve the respiratory obstruction. He was posted for elective tracheal stenting and dilatation one week later. On spontaneous breathing, a tube exchanger was introduced through the tracheostomy tube; A Flexometallic endo tracheal tube was threaded over the tube exchanger through the tracheostomy port; tracheostomy tube was removed; patient was paralysed and intermittent positive pressure ventilation was established through the flexometallic tube. At the end of the stenting procedure, spontaneous breathing was re-established; tube exchanger was introduced t hrough the ETT; oxygenation was continued; flexometallic tube was removed; tracheostomy tube was reintroduced over the tube exchanger and tube exchanger was removed. Since management of airway and ventilation is a challenge during procedures involving airway, like tracheal stenting, it has to be carefully approached during induction, during the surgery, at emergence and in the post-operative period.
