Management of diabetic foot ulcer: A comparative study between pentoxifylline and traditional therapy

Abstract
Background: Foot ulcers are a major complication of diabetes and are associated with a substantial burden for the patients and the entire health care system. The role of wound care is crucial in the management of diabetic ulcers. An ideal wound care product in addition to controlling the infection should also protect the normal tissues and not interfere with normal wound healing. Pentoxyfylline is known to decrease the rouleaux formation of RBC and hence helps in improving the microcirculation. Objective: To assess the effect of pentoxifylline in treatment of diabetic ulcer compared with other traditional treatment aspects. Methodology: This is a prospective study conducted in surgery department during August 2010 to May 2012 at Aarupadai Veedu Medical College and Hospital, Pondicherry including 62 patients having diabetic foot ulcer. Group A receiving traditional treatment i.e. bed rest with elevation, antibiotics, analgesics and dressings and Group B receiving pentoxyfylline along with traditional treatment. The response was observed subjectively, objectively, on histological basis, visually and by Doppler. Results: Mean improvement in blood velocity in patients treated with pentoxifylline was 0.21±0.03 and that of other patients were 0.09±0.04 (<0.05). Patients treated with pentoxifylline shows good improvement in healing as compared to patients who received no treatment. 73.33% were treated with pentoxifylline showed presence of minimal slough on Day 30 as compared to 43.75% in other group.(<0.05). Conclusion: Pentoxifylline gives better results in healing of diabetic foot ulcer as compared to traditional treatment.
