Measurement of HCG - diagnostic dilemmas

Abstract
Human chorionic gonadotropin (hCG) is produced by placental syncytiotrophoblast. Its detection confirms pregnancy. There are many variants of hCG which causes problem in proper detection. Upto five weeks of pregnancy hCG-h (hyperglycosylated hCG) is predominant in urine. After five weeks hCGβcf (β core fragment hCG) is predominant in urine. But many of the test devices cannot detect hCG-h or hCGβcf, hence, resulting in false negative result and its worse consequences. Forming better devices, serum estimation, urinary dilution, following WHO IRRs (International Reference Reagents) are some solutions to overcome this problem and increasing diagnostic accuracy.
