Mediators of inflammatory and coagulation processes in acute pancreatitis – selected issues

Abstract
In the severe form of acute pancreatitis (AP) both, local and systemic inflammatory response reflects the interaction between activated endothelial cells (ECs) and leucocytes, thrombocytes, mast cells and released by them mediators. Increasing inadequate and not properly orientated inflammatory response, in the form of systemic inflammation (SIRS) and coagulopathy, leads to the development of multiorgan insufficiency in AP. A salient role in this process is attributed to proinflammatory cytokines (TNFα, IL-1, IL-6, IL-8, IL-18), whose activity leads to impairment of the compensatory antiinflammatory response syndrome (CARS), dependent of the antiinflammatory cytokines (IL-10, IL-2, IL-4, IL-1ra), and adhesive molecules accountable for selective recruitment of circulating leukocytes subpopulations to the inflammation site. The early commencement of endothelial coagulation factors cascade, especially tissue factor (TF) activation, is an integral component of this process. In this review the published results of the authors’ studies have been presented.
