Morphological analysis of coronary atherosclerosis by modified American heart association classification in young individuals- An autopsy study

Abstract

Background and Objectives: Coronary artery disease has emerged as new epidemic affecting Indians at relatively younger age with severe and diffuse form of disease. This autopsy study was conducted in young individuals to assess the morphological changes in coronary arteries and classify the atherosclerotic lesions by using modified American heart association (AHA) classification and to estimate prevalence of coronary atherosclerosis. Materials and Methods: Hundred heart specimens of subjects aged 15 to 34 years were included in study. Sections from proximal and distal part of left anterior descending artery, left circumflex artery and right coronary artery were given. Microscopic typing of atherosclerotic lesions was done by modified AHA classification. Statistical analysis was done and chi-square test was used to see the association. Results: Out of 100 cases, 65% were males and 35% were females. Among 54% cases with atherosclerosis, 19% cases showed advanced lesions. The prevalence and severity of atherosclerosis was more in males (63.04%) compared to females (37.14%). Left anterior descending artery was most commonly involved by atherosclerosis. Intimal xanthoma was most common lesion overall. Pathological intimal thickening was most common atherosclerotic lesion. Fibrous cap atheroma was most common advanced lesion. Thin fibrous cap atheroma and inflammation which are indicators of vulnerable plaques, were significantly more in males compared to females. Conclusion: The modified AHA classification has more clinical implications and offers better categorization of atherosclerotic lesions. The increased prevalence of atherosclerosis (advanced and intermediate lesions) found in young population gives an indication that preventive measures need to be implemented.
