Morphological study of placenta in preeclampsia and its association with natural killer (NK) cells

Abstract

Background and Objectives: Preeclampsia (PE) is a hypertensive disorder of pregnancy characterized by deficient placentation due to inability in remodeling the uterine arteries. A variety of morphological changes are known to occur in placenta in PE. Very few studies have been done in India on NK cells in preeclamptic placenta. Materials and Methods: In the present observational study a total of 76 placentae were collected, of which 38 were from women with diagnosis of PE and 38 placentae from normal term pregnancies. The objectives of the study were to assess the morphological features and expression of decidual NK cells by CD56 immunostain in placentae of patients with PE and to compare them with the placentae from normal term pregnancies. Placentae collected were grossly examined. Hundred villi from minimum four slides were counted and results were recorded. IHC stain was done by CD56 antibody on the tissue block of placental disc containing decidua basalis. NK cells were counted per high power field and scored. Statistical analysis was performed using SPSS software version 22. Mann-Whitney U-test was used to compare the differences between the control group and women with PE. For all statistical analysis P value < 0.05 was considered significant. Results: The mean trimmed weight and diameter of placenta was significantly less in preeclamptic placenta. The significant findings were increased area of maternal surface infarcts, fibrinoid necrosis, occlusion of the fetal artery, syncytial knots, smooth muscle hypertrophy of spiral artery and thickening of basement membrane of villi. There was significant increase in the number of CD56 positive decidual NK cells in PE group. Conclusion: Morphological study of placenta and NK cell status play a vital role in planning and management of future pregnancies.
