Prevention of pain on propofol injection in children: Comparative study of pretreatment with ketamine, tramadol and normal saline

Abstract

 BACKGROUND: In children the pain is very complex phenomenon due to misconception that they do not suffer the physical and psychological raveges of pain but now a days pain relief in children has gained more popularity as a part of quality health care. Propofol is a worldwide newer drug used in Anaesthesia for children but it has only one disadvantage of producing pain on injection which can be overcome with pretreatment with ketamine or propofol. Aim of study: To compare the effect of tramadol and ketamine on incidence and severity of pain on injection of propofol in children. MATERIALS AND METHODS: A prospective randomized study was carried out in 90 children of ASA physical status I and II undergoing elective or emergency surgery under General anaesthesia. The children were randomly divided into three groups Group I,II,III received Ketamine, Tramadol Normal Saline before propofol injection respectively. All the patients were given conventional general anesthesia. No analgesics were given in premedication propofol pain was observed using four point behavioral scale and hemodynamics were observed at different time intervals. Statistical Analysis: Results were compared using the students unpaired„t‟ test after calculation of mean, standard deviation, and standard error of mean in all the three groups. RESULTS: There was significant rise in pulse rate and blood pressure from basal value to at the time of propofol injection in group III compared to group I and II. There was also significant reduction in pain after propofol injection in Group I and II. CONCLUSION: Intravenous ketamine and tramadol pre-treatment significantly reduces the incidence and intensity of pain on injection of Propofol, however ketamine appears to be a better than tramadol in reducing Propofol injection pain
