Nocardiosis In an Immunocompetent Host

Abstract
The culprit for Nocardiosis is an aerobic actinomycetes in the genus Nocardia, a rare gram- positive bacteria. The most common disease sites include lungs, central nervous system, and skin. CNS involvement is usually present in 20% of patients. It can affect any area of the brain. It may be mistaken for primary or metastatic brain cancer prior to availability of tissue diagnosis. Diagnosis of Nocardiosis can be challenging despite their known propensity for brain and lung involvement. We describe a patient who presented with a multitude of neurological symptoms and was found to have pulmonary and cerebral abscesses due to Nocardia by virtue of brain biopsy. This patient was treated with intravenous trimethoprim-sulfamethoxazole for a month and was switched to oral trimethoprim-sulfamethoxazole for 11 months. His hospital course was complicated by hydrocephalus that required shunt placement. We reviewed this case in view of the current literature.
