Obesity – is always primary?

Abstract
Secondary causes of obesity (few percent of all obesity cases) are endocrine and genetic disorders, central nervous system leasion or medication taken by the patient. Based on clinical picture it is usually possible to distinguish secondary causes of obesity, but sometimes the signs of secondary obesity are discreet or atypical. The diagnose can be established by taking a patient history, performing a physical examination and (in some cases) laboratory tests, which depend on symptoms, risk factors and index of suspicion. In ambulatory practice in the obese patients should be excluded: hypothyreosis (testing TSH), in some cases (with signs of hypercortisolism) Cushing syndrome and in obese women (with menstrual disturbances and hyperandrogenic symptoms) polycystic ovary syndrome. Among obese children and adolescents these with dysmorphic features, short stature, development or puberty delay and other endocrine abnormalities should be refered to the specialistic (endocrinology or genetics) clinic.
