Position of ablation in the management of arrhythmias

Abstract

Ablation of supra- or ventricular arrhythmias is well known and widely used procedure. The method is based on delivery to the connective tissue the high (heating) or low (freezing) temperature, and in a result damage of that region of the heart. Ablation is a first line therapy with the high success rate and low complication rate of supraventricular arrhythmias (atrio-ventricular nodal reciprocating tachycardia, accessory pathways, atrial flutter or atrial tachycardia). Ablation (pulmonary veins isolation) is also widely used in atrial fibrillation patients. In ventricular arrhythmias in patients with structural heart disease ablation is an additional therapy, following ICD implantation and pharmacological treatment. In patients without structural heart disease ablation reduces palpitations and risk of development of arrhythmia induced cardiomyopathy. Electroanatomical systems reduce fluoroscopy and simplify mapping of supra- or ventricular reentry circuits.
