Powikłania oczodołowe w przebiegu leczenia grzybiczego zapalenia zatok u pacjenta po przeszczepie serca

Abstract

On the one hand the develo pment of trans plantation is an opp ortunity for patients, but on the other hand – a challenge for the healthcare system. Patient after trans plantation, especially in result of immunosuppressio n, is exposed for opportunistic infections, including the fungal sinusitis. In the literature there are only a few reports on invas ive sinusitis in people after heart trans plantation, in the opposed to many retrospective series studies of seve ral hu ndred patients suffe ring from chronic sinusitis in the course of leukemia or a certain nu mber of paper s concerning the complica tions of sinuses after kidney and – less often – liver trans plantation. A 41-year-old man with six years after autologous bone marrow trans plantation due to Hodg kin's disease, and four years after heart transplantation because of drug-induced cardiomyopathy, underwent ethmoidectomy as the treatment of fungal sinusitis. In the postoperat ive period there were two orbital complication s: abscess of the right orbit on the first day after surgery and preorbital abscess at the same side, on the sixth day afterthe initial treatment. Abscesse s were successfull y drained , doses of immu nosuppressive drugs were temporary reduced, and the patient was discharg ed home in good general condition. Many factors can affect the occurrence of complication s in the trea tment of sinusitis in the case of org an transplant. A multitude of problems of such patient requires special multidisci plinary care and individualized trea tment that should be performed in experienced tertiary center.
