PROBLEMS OF LOW CARDIAC REHABILITATION PARTICIPATION AT A TERTIARY HOSPITAL - A REVIEW

Abstract

India has the dubious distinc tion of being the heart disease (HD) and diabe tes capi tal of th e world. HD is t he l eadi ng caus e of de at h and t he wo rst par t is tha t i t strikes Indians during the prime y ea rs of th eir life. Th e HD is a larg ely pr eve nt able disease a nd if ap pro pri at e lifestyle m easu res, medica tion an d app ro pria te p hysical exercises ar e tak en e arly enou gh on e may nev er ne ed to u nd er go t he emo tional tu rmoil. Cardiac Rehabilitation (CR) pro gram is for those who have alr ea dy suffe red a c ardi ac eve nt, such as a he ar t at tack, byp ass surge ry or an gioplas ty. CR is an inte rven tion t ha t has a w ell-docum ent ed history of be nefi ting pa tie nts. Th ese b en efits includ e improvements in func tional capacity, quality of life, and morbi dity and mort al ity. Red uctions in morbi dity a nd m or tal ity wit h CR ar e e quival en t to many of t he b est available pha rm acologic an d invasive int erv en tions. The CR is Class-I-l evel recomme nd ation (signifyin g a tr ea tme nt t ha t shoul d be perf orm ed o r admi niste red ) for ref er ral to CR in t hose g rou ps of pa tie nts men tione d above. Y et CR p ar ticipatio n a nd r efe rr al h as no t i ncre ase d ove r the last decade; a time during which invasive interventions (largely PCI) have skyrock et ed, d espite th e obse rva tion t ha t P CI d oes no t imp rove outcom es. Firstl y, the re ason was cost. The gove rnm en t schem es in Kar na tak a a pplicable t o th e te r tiary hospi tal like yeshaswini, BPL, A PL car d and th e insu ra nce com pa nies n ee ds to inclu de th e C R p ro gr am a t a cost so tha t al l type of ca rdiac p ati en ts can be nefi t which is now l ag gin g. The ben efits of CR ar e g re atly u nde r-ap pr eciat ed i n bo th the pu bl ic and th e medical comm unity in this pa rt of t he worl d. G re at er effo rts ar e ne ed ed to educ at e he alth ca re p rovide rs and t he p ublic reg ar din g th e ben efits of CR. Hence e duc atin g th ese he alth ca re prof essionals, ot he r h eal th c ar e providers, pati ents, families, health care systems regarding the indisputable benefits of multidisciplinary CR will go a long way toward broadening the prop or tion of p atie nts w ho r eceive thes e be nefi ts
