Prosthetic rehabilitation of the edentulous mandible with a bolt-retained overdenture immediately loading implants. A case report

Abstract

Prosthetic rehabilitation improves the comfort of denture usage in the edentulous mandible by means of overdenture restorations supported by implants. A satisfactory retention is provided by a combined implant design, magnetic retention, coupling ball or telescopic crowns. Frequently, two implants are symmetrically spaced in the anterior area in the position of teeth 33 and 43. The aim of this study was to assess the effectiveness of the rehabilitation method involving an overdenture restoration supported by immediately loading implants combined with a beam with a bolt retention system. The implantation was performed using a surgical template prepared in advance. It was stabilised in the oral cavity with pins. The bed for implants was prepared through the holes in the pattern placed in the template, where next Neoss implants were placed with a diameter of 4.0 mm and a length of 17 mm. The implants were combined with a beam with a hole in its axis, which corresponded with the lock positioned in the matrix fixed in the prosthesis. The patient received a key to open the bolt and was instructed on the operation of the mechanism. Two and a half years after the implantation the patient was satisfied with the prosthesis, especially the overdenture with the bolt retention system that provided its very good stability. The examination confirmed a very good stability of the implants and a proper work of the retentive elements. The patient maintained a satisfactory oral and retentive elements hygiene. There were no inflammatory changes in the oral mucosa around the implants.
