Psychogenic movement disorders

Abstract
Psychogenic movement disorders (PMDs) are characterized by abnormal motor behaviors. These may resemble organic tremor, dystonia, chorea, tics etc. The spread of movement is often nonanatomical. Distractibility reduces or resolves it. PMDscan occur at any age. Women are more often affected. There is often the history of the psychical or physical abuse. Abrupt onset is characteristic.PMDs represent a diagnosis of exclusion. Certain characteristics in the history or examination are the most important in the differential diagnosis. There is no standard set of diagnostic tests that are indicated in all types of the PMDs. Two sets of clinical criteria, the Fahn and Williams/Shill and Gerber, have been developed. Their reliability is insufficient. There is no standardized pharmacological treatment for the patients with PMDs. Other interventions such as psychotherapy, lifestyle changes, moderate exercises and healthy diet should be considered. The aim of the therapy should be decreasing of the disability of the patient, increasing of the quality of life, unemployment reduction and long term remission. Negative prognostic value has been associated with long-standing symptoms, diagnosis of personality disorders or somatic disease, low educational level.
