Revalence of the Symptomatic Cardiovascular Diseases during the Pregnancy and in Post Partum in Parakou’s Hospital Settings in 2015.

Abstract
Introduction: L’incidence des affections cardiovasculaires au cours de la grossesse est mal connue surtout en Afrique. Ce travail vise à étudier la prévalence des pathologies cardiovasculaires au cours de la grossesse et dans les suites de couches en milieu hospitalier à Parakou en 2015.Méthodes: Il s’agit d’une étude prospective transversale descriptive à visée analytique allant du 01 Mars 2015 au 31 Juillet 2015. Les parturientes et gestantes suivies en consultation ou hospitalisées au Centre Hospitalo-universitaire et Départemental du Borgou et à l’Hôpital d’Instructions des Armées de Parakou présentant au moins un signe d’appel cardiovasculaire ont été systématiquement recensées. Le diagnostic des différentes affections cardiovasculaires au cours de la grossesse a été fait conformément aux normes internationales.Résultats : Sur 2512 parturientes et gestantes admises, 180 avaient une pathologie cardiovasculaire soit une prévalence de 07,16%. L’âge moyen était de 28,55 ± 6,86 ans. Les pathologies cardiovasculaires rencontrées étaient  l’hypertension artérielle (HTA) avec 155 cas (86,11%) dont 61 (33,88%) de pré-éclampsie, 56 (31,12%) d’HTA gestationnelle, 15 (08,33%) d’HTA chronique et 23 (12,78%) d’éclampsie ; 21 (11,67%) de cardiomyopathie du péripartum, 03 (01,67%) de myocardiopathie hypertensive et enfin 01 (00,55%) de valvulopathie d’origine rhumatismale. Les facteurs associés à l’HTA étaient le jeune âge ≤30 ans (p=0,007), la primigestité (p=0,0007) et la nulliparité (p=0,000002). Ceux associés à l’insuffisance cardiaque étaient l’âge ≥30ans (p=0,001), l’ethnie (p=0,0005), la multigestité (p=0,0002) et la multiparité (p=0,00000009).Conclusion: A Parakou, les affections cardiovasculaires au cours de la grossesse sont fréquentes et variées. Des activités de sensibilisation doivent être menées autant auprès des agents de santé pour favoriser un meilleur dépistage de ces affections.Mots clés: Maladies cardiovasculaires - Hypertension artérielle – Insuffisance cardiaque – Grossesse - Afrique.SUMMARY:Introduction: The incidence of cardiovascular diseases during pregnancy is poorly known in Africa. Our purpose was to estimate the prevalence of cardiovascular diseases during pregnancy and in the post partum in Parakou’s hospital in 2015.Methods: This is a prospective cross-sectional descriptive analytical purpose from March 01, 2015 to 31 July 2015. Parturient women and pregnant followed in consultation or hospitalized at the Centre of Borgou and University College Hospital and the Hospital of Instructions of the armies of Parakou with at least a sign of cardiovascular appeal have been systematically recorded. The diagnosis of various cardiovascular diseases during pregnancy has been done according to international standards.Results: 2503 parturients and pregnant admitted, 180 had a cardiovascular pathology with a prevalence of 07,19%. The average age was 28.55 ± 6.86 years. Cardiovascular pathologies encountered were hypertension (HTA) with 155 (86.11%) including 61 (39.35%) of pre-eclampsia, 56 (36.13%) of HTA gestational, 15 (09,68%) of HTA chronic and 23 (14.84%) of eclampsia; and finally 25 of cardiopathy with 21 (84%) of peripartum cardiomyopathy (PPCM), 03 (12%) of hypertensive cardiomyopathy and 01 (04%) to valvular disease of rheumatic origin. Factors associated with hypertension were the young age ≤30 years (p =0,007), the low socioeconomic level (p = 0,02), the low gestity (p =0,0007) and the low parity (p =0,000002). Those associated with PPCM were age advanced ≥30ans (p = 0,001), ethnicity (p =0,0005), the non-schooling (p =0,02), the low socioeconomic level (p =0,02), multigestity (p = 0,0002) and multiparity (p =0,00000009).Conclusion: In Parakou, cardiovascular diseases during pregnancy are frequent and varied. Awareness-raising activities should be conducted with health and women workers in order to prevent these conditions.
