Splenic Preservation in Blunt Pancreatic Trauma – Report of a Case

Abstract
The management of pancreatic trauma is challenging and is essentially dictated by the status of the main pancreatic duct. Lesser grades of injuries without ductal involvement usually respond to conservative management, but once the main duct is involved, patients usually need some form of intervention. For patients with injuries to the body and tail of the pancreas, the classical operation has been distal pancreatectomy (with splenectomy). With a better understanding of splenic functions and an awareness of the dangers of the asplenic state over the last few decades, the concept of splenic conservation has led to a change in surgical strategy, with spleen preserving distal pancreatectomy (SPDP) being offered in the setting of pancreatic trauma in hemodynamically stable patients.
