Study and management of locally advanced breast cancer

Abstract

Objective: To study and management of locally advanced breast cancer. Design: A prospective study in the management of locally advanced breast cancer. Results: The management of locally advanced breast cancer is complex. Regardless of the situation of the in which the disease has become apparent, the patient should be fully evaluated and management should be tailored individually. Chemotherapy, either neoadjuvant or postoperative should be an essential part of management for each and every patient. Patient with good response to neoadjuvant chemotherapy have better prognosis and chance for long term disease free survival. Response to neoadjuvant chemotherapy with CAF regimen is better than with CMF regimen and should be offered to the patient whenever feasible. Surgery still remains the corner stone in management of stage III breast cancer patients and this is true even in those who had complete response to neoadjuvant chemotherapy. Patient undergoing modified radical mastectomy have much better loco regional control of the disease. Radiation therapy should also be an integral component in the management of locally advanced breast cancer for optimal locoregional control. Cure is an unrealistic expectation in advanced breast cancer but multidisciplinary approach can give patient long-term disease free survival with more so with good quality of life.
