Subklinik Hipertiroidinin Bir Yıllık Seyrinin Değerlendirilmesi

Abstract
Aim: We aimed to evaluate the possible clinical changes in subclinical hyperthyroidism determined by thyroid stimulating hormone, free triiodothyronine and free thyroxin levels during one-year follow-up. Methods: In Haseki Treatment and Research Hospital, patients who had assessed by thyroid function tests during the first 6 months of 2014 were categorized based on their thyroid function test results. 27361 patients with either euthyroidism or subclinical hyperthyroidism were separated. 362 cases who had at least two thyroid function test results in relevance with subclinical hyperthyroidism were included in the study. Age range was 18 to 82 years. Those 362 patients were called by phone and asked to visit the internal diseases department for further physical examination, electrocardiogram and blood tests. Detailed anamnesis and physical examination findings were recorded in 38 patients who visited the hospital on our call. Thyroid ultrasonography results, operation and drug use history were evaluated. Results: Patients were divided into three groups according to thyroid stimulating hormone values: euthyroid, mild subclinical hyperthyroidism and severe subclinical hyperthyroidism. Age, gender and metabolic parameters were evaluated. Between groups there were statistically significant differences regarding age, gender, blood glucose, blood cholesterol and LDL values. Disease progression to clinical hyperthyroidism was statistically significant, especially in patients followed for more than 6 months. Conclusion: Early treatment of subclinical hyperthyroidism is important in order to prevent its possible cardiac and osteoporotic complications. Patients with severe subclinical hyperthyroidism should be followed up closely for more than 6 months.
