Talon cusp on mesial upper incisor – a case report

Abstract
[b] Introduction[/b]. A talon cusp represents a developmental disorder of tooth anatomy and it includes regularly structured enamel and dentine, and in certain cases also a pulp horn of different sizes. [b]Case report[/b]. A 6-year- and nine-month-old patient presented to the Chair and Department of Jaw Orthopedics at the Medical University of Lublin. During an intraoral examination, an anomalous upper central incisor 11 was found. It had a long, talon cusp protruding from the palatal surface and reaching the incisal edge. The labial surface of tooth 11 also showed an irregular structure. [b]Treatment history[/b]. In the first stage, a glass-ionomer cement wedge was used on tooth 36 and 46. Next, a removable upper appliance with bite planes was made to enable further eruption of tooth 11, and a lower removable appliance as well. At the same time a gradual grinding of the talon cusp was started and then such procedure was continued every three months. Then preparations with a high fluoride concentration were used. Such treatment procedure enabled eruption and proper location of tooth 11 in the dental arch without any loss of its vitality. Further treatment plan will include correction of tooth 11 labial surface shape , a reduction of its increased mesio-distal size and periodical radiological follow-up. [b]Discussion[/b]. A talon cusp typically causes problems related to occlusion, speech and aesthetics. If such problems occur, two alternative methods of treatment can be used: a gradual reduction or complete removal of such cusp during a single visit, combined with endodontic treatment if necessary.
