The gastroenterological manifestations of methotrexate effects in pediatric patients with the juvenile idiopathic arthritis

Abstract

The paper elucidates the most prevalent gastroenterological manifestations of methotrexate effects in the treatment of juvenile idiopathic arthritis (JIA) in pediatric patients. Some considerable changes took place in the JIA treatment over the past decades, but methotrexate remains the key drug for JIA treatment. Methotrexate is a first-line medication among synthetic disease-modifying antirheumatic drugs, which in the absence of contraindications is prescribed immediately after the diagnosis, in the presence of unfavorable prognosis factors and moderate or high inflammatory activity. The gastrointestinal manifestations of methotrexate effects in JIA treatment are being considered as not only factors, aggravating patient’s quality of life, but also complicating further possibility of using this medication in JIA treatment. The main mechanisms and clinical manifestations of the adverse effects of the drug have been analyzed. The data of reported cases of nausea and vomiting during the use of methotrexate for JIA treatment in the world have been presented, they predetermine further study of their causes and mechanisms of developmental. The most important thing is to prevent irreversible complications of JIA treatment with methotrexate, but with this it is necessary to concentrate on the prophylaxis of possible gastroenterological complications, including drug-induce liver injury, and prevent formation of the non-alcoholic fatty liver disease (NAFLD). NAFLD covers a wide range of pathological changes, ranging from steatosis to more severe forms of NAFLD, such as non-alcoholic steatohepatitis, fibrosis, cirrhosis and hepatocellular carcinoma. The authors presented data of the international trials, that proved actuality of this problem as well as the efficacy of measures to prevent the adverse events in the course of JIA treatment.
