The impact of the improvement of the McKenzie method on the quality of life in patients with L-S discopathy after surgery – case report

Abstract

[b]Background.[/b] Pain syndromes of the lumbosacral spine are a big problem, not only in elderly individuals, but also in the fully active young and middle aged working population, due to the pain and functional limitations. They are one of the leading causes of absenteeism and disability and have a major impact on the quality of life, often leading to its deterioration. Eligibility for surgery and its implementation does not always relieve pain in suffering patients. The aim of this study was to evaluate the efficacy of Mechanical Diagnosis and Therapy by McKenzie (MDT) in post-surgical rehabilitation and the impact of this approach on the improvement of the patient’s quality of life.[b]Case study. [/b]A patient with chronic back pain which was recurrent and intensifying during prolonged sitting and walking was studied. Due to the persistent pain the patient underwent a surgical procedure in 2010 which involved dynamic interspinous stabilization between the L4 and L5 vertebrae. Although the surgery decreased the pain, it did not significantly improve the patient’s subjective assessment of his quality of life. Taking into consideration the patient’s personality, occupational and non-occupational activities and the willingness to return to normal functionality, it was decided to subject the patient to rehabilitation using McKenzie approach. [b]Results.[/b] The conducted rehabilitation, according to the postoperative procedure algorithm using McKenzie approach made it possible for the therapists to achieve therapeutic progress, a significant reduction in pain and to restore the full function of the lumbar spine with full mobility, thereby increasing the quality of life of the patient who was able to return to work.[b]Conclusion.[/b] An accurate diagnosis based on the analysis of variability of the symptoms evoked in repeated movement test (typical for McKenzie approach only) enables appropriate treatment through the application of optimal therapeutic procedures, resulting in a significant improvement of the patient’s quality of life.
