The role of polymorphonuclear granulocytes in transplanted kidney ischemia-reperfusion injury development.

Abstract

Ischemia-reperfusion injury (I/R injury) is a non-specific, antigen-independent tissue-destructive process affecting both early and long-term function of the allograft. Existing studies amply document pathophysiology of I/R injury and attribute it largely to neutrophil activation. Both experimental and clinical data confirm that a consistent feature of tissue injury following ischemia and reperfusion is acute inflammatory response with neutrophil infiltration and activation of vascular endothelium. Neutrophils (PMNs) are mediators of I/R injury following deceased-donor renal transplantation (kTx). The mechanism of tissue destruction by neutrophils involves production of reactive oxygen metabolites as well as secretion of proteolytic enzymes, such as serine proteases.
