Titanium Elastic Nailing System (TENS) In Paediatric Shaft Femur Fractures: Our Experience In 20 Cases

Abstract
Background: Femoral Shaft fractures in paediatric age group are common injuries and needs prompt intervention. The results mainly depend on the type of implant used and good anatomical reduction. Use of Flexible intramedullary nail has given promising results in paediatric population Aim: To assess the outcomes of paediatric femoral shaft fractures treated with TENS system .the results are graded using Flynn’s criteria Materials and Methods: This is a retrospective study of prospectively collected data of paediatric shaft femur fractures treated with TENS system. The patients were evaluated with pre-operative radiographs. All patients underwent surgery with TENS nails after appropriate reduction. The patients were followed up at regular intervals, radiographs were assessed for fracture union. The results were assessed with Flynn’s criteria. Results:A total of 20 patients were included in the study. The mean age of the group was 8.25 years with 18 boys and 2 girls(M:F-9:1).The average weight of the patients was 25.05kgs(15-36kgs).Middle 1/3 rd fractures was most common with seven cases of transverse pattern and seven each of short oblique and spiral fractures. The mean duration of full weight bearing was 10.6 weeks and mean duration of radiological union was 11.89 weeks. Two patients(10%) had pin site irritation and one patient(5%) had deep infection. Seventeen patients 85% had excellent , 2(10%) had satisfactory and one patient (5%) had poor outcomes according to Flynn;s criteria. Conclusions:Titanium Elastic Nailing System(TENS) is safe and an effective treatment modality for femur shaft fracture in children in the age group of 5 – 12 years. Faster fracture healing, early mobilisation, faster rehabilitation in children are promising results with flexible intramedullary nails.
