Urological Manifestations of HIV infection and AIDS

Abstract
Introduction: Global scenario of HIV infection has changed dramatically of late. The increasing availability of antiretroviral drugs has helped to combat HIV replication. Both viral load and CD4 T-cell count are reliably used to monitor and prognosticate disease progression. Urologic complications are often the first clinical signs of HIV infection. The aim of this study was to evaluate the incidence rates of the urological manifestations of HIV infection & AIDS. Materials and Methods: This was a cross sectional hospital based study conducted in our institute. The study population were HIV positive cases. A predesigned proforma was used to record the patient particulars & various parameters. A thorough clinical history, physical examination and relevant set of investigations performed. The incidence rates of the various manifestations were correlated with the CD4 count, ART intake etc. The data obtained from the study was statistically analysed using SSPS version 20. Results: A total of 131 HIV positive patients (M: F = 3:2) were enrolled in this study. Most of the housewives got infected from their high risk husbands by sexual route. The asymptomatic patients were on regular ART regimen. The clinical spectrum ranged from being normal to as severe as Fournier’s gangrene. UTI was seen in 40% patients. The most common surgical procedure attempted was DJ stenting. Conclusion: Routine screening for the entire patient population must be the standard of care. Both viral load and CD4 count reliable parameters to predict urinary manifestations of HIV. There is a need for creating an awareness of the urologic manifestations of HIV and AIDS among the population in general and the HIV infected people in particular.
