A cytological study of salivary gland lesions with fine needle aspiration

Abstract
Background:A mass in the salivary gland region often are a diagnostic challenge. The present study was conducted to study the non-neoplastic and neoplastic lesions of the enlarged salivary glands by FNAC in patients presenting with salivary gland enlargement to determine the pattern of disease affecting salivary glands and to study the spectrum of lesions with respect to age, sex and site of occurrence.Methods: A total of 90 FNACs were done on salivary gland swellings from November 2016 to April 2018 at the Department ofPathology, East Point College of Medical Sciences and Research Center, Bangalore. One or more smears made from the aspirated material were fixed immediately in 95% ethanol/Carnoy’s fixative and stained with hematoxylin and eosin and Papanicolaou stain. One or more air-dried smears were stained with MGG stain. PAS stain was done on methanol fixed smear, if required.Results: Out of the 90 cases, 53 cases were diagnosed as non-neoplastic lesions and 37 cases as neoplasticlesions.The majority of the patients were in the male group gender of 21-30 years. In the presentstudy, the parotid gland (58 cases) wasthe commonestsite involved.Pleomorphic Adenoma (23 out of 30 benign tumors) and Mucoepidermoid carcinoma (4 out of 7 malignant tumors) were the commonest benign and malignant tumors respectively.Histopathological correlation was available in 30 cases. Out of which 22 cases were true positive, 2 cases were false positive, 1 case was false negative and 5 cases were true negative.Interpretation and Conclusion: The overall Sensitivity, Specificity and Diagnostic Accuracy were 95.65%, 71.43% and 90% respectively in the diagnosis of salivary gland tumors. Hence FNAC of the salivary gland lesions is accurate, simple, rapid, inexpensive, well tolerated and has high Diagnostic Accuracy and helps in early appropriate therapeutic management.
