AUTOLOGOUS FAT TRANSPLANTATION TO THE BREAST ASSOCIATED WITH BRAVA SISTEM: 5 YEARS RETROSPECTIVE STUDY

Abstract

Autologous fat transfer is a surgical technique commonly used for correcting soft tissue defects, but also for aesthetic purpose. In the hands of an experienced surgeon this technique is easy to perform and has few complications. We performed a retrospective study by analysing 34 patient’s files who had autologous fat transfer for breast reconstruction/augmentation during the period 2012-2016 in Plastic & Aesthetic Surgery Center Iasi. The AFT was performed on 42 breasts for: congenital breast abnormalities (tuberous breasts, Poland Syndrome, micromastia), post-mastectomy, aesthetic augmentation and capsular contracture. Lipomodelling was usually made under general anaesthesia. Fat was harvested from the abdomen, infragluteal fold, and the lumbar region exceptionally, depending on the patient’s natural fat deposits. After harvesting, the fat was centrifuged to remove blood, infiltration solution, old fat cells and debris. In the cases where the skin was insufficient or with a doubtful quality, the BRAVA external expansion system was used pre and postoperatively. Clinical follow-up shows that the results were highly satisfactory for both patients and surgeons with a satisfaction rate of 95%. The patients were followed up at 15 days, 3 months, 6 months and 1 year postoperatively. At each postoperative consultation, photos were taken and the size, shape and symmetry of the reconstructed breast were analysed. Complications were found in 7.8% of cases and included: 3 cases of liponecrosis, 2 cases of hyperchromic scars (due to long-term use of the BRAVA system). If breast changes have occurred, ultrasounds and/or mammograms have been used. In liponecrotic cysts, the simple aspiration puncture was enough, and the suspicion of breast cancer was denied. Lipofilling, presents a good option for plastic, reconstructive and aesthetic surgery of the breast. The addition of Brava expansion system leads to significantly larger breast augmentations, with, higher graft survival rates, and minimal complications. Although AFT seems to be a simple procedure, it should be done only by experienced, well-trained surgeons to minimize fat resorption or complications, demonstrating high safety and efficiency of the procedure.
