Behavioural cardiology - the knowledge advances outline and clinical applications in the study of the coronary artery disease

Abstract
The aim of this study is to discuss the advances in research concerning the role of psychosocial factors in the etiopathogenesis and course of coronary artery disease, with the emphasis on the possibilities of the clinical applications. The key frame of reference in the analysis will be the work results of the international research team Psychocardiology Status Conference, where the knowledge advancement concerning this issue was verified and validated with the application of rigorous methodological criteria. The research results of Psychocardiology Status Conference created a reliable basis for evidence-based behavioural cardiology, the new discipline which analyses the role of psychosocial factors in the area of interest of clinical cardiology concerning the issue of coronary artery disease. In their works Psychocardiology Status Conference stressed the significant role of a low socioeconomic status, low social support and chronic work stress as well as depression in the etiopathogenesis of coronary artery disease. The type A behavioural pattern, however, was questioned as a coronary risk factor. Nowadays the key research point in behavioural cardiology is the issue of psychosocial intervention effectiveness, estimated with respect to the mortality rates and the reoccurrence of acute coronary events. In the light of the negative results concerning this issue obtained in the research programmes ENRICHD and SADHART, creating the model of secondary prevention of coronary artery disease which takes into consideration the psychosocial coronary risk factors encounters major difficulties.
