Bezpieczna zamiana inhalatorów suchego proszku (DPI)

Abstract
Interchangeability of DPI’s at the level of the pharmacy or by other health providers is a fact. The decision to do this usually falls apart from the initial health provider who prescribed the initial inhalation therapy. The interchangeability of DPI’s is especially important due to the fact that each DPI should be individually dispensed to the patient. In order to explain this complicated issue the process of generating a medicinal aerosol is discussed in detail, with emphasis on aerozolization, deaggregation, penetration and deposition, all depending on the inspiratory maneuver of the patient. This detailed complex process underlines the need to carefully assess any decision to interchange a DPI, especially in patients with very low inspiratory flow (young children, acute obstructed, asthma and COPD patients). Registration processes of generic DPI’s with emphasis to the interchangeability of such devices in discussed. The detailed process of making such a decision is shown on the example of the Easyhaler® DPI, which has proven to be interchangeable in such cases. ............................... Zamiana jednego inhalatora produktu leczniczego na drugi jest coraz bardziej powszechna. Decyzja o takiej zamianie często zapada poza kontrola lekarza pierwotnie planującego terapię jak i samego chorego. Szczególnie trudne jest to zagadnienia w przypadku zamiany jednego inhalatora suchego proszku (DPI) na drugi. Dla pełnego zrozumienia istoty problemu zamiany przedstawiono dokładnie proces powstawania aerozolu leczniczego w przypadku DPI. Uwzględniono procesu aerozolizacji, deagregacji i deponowania cząstek leczniczego aerozolu. Omówiono wpływ oddechu na proces generowania aerozolu. W skrócie przedstawiony sposób generowania aerozolu przez DPI podkreśla trudności w swobodnej zamianie inhalatorów suchego proszku. Omówiono na przykładzie inhalatora Easyhaler® jakie warunki musi spełnić inhalator aby mógł być swobodnie zamieniany na inny.
