Comparing efficacy of retrograde intrarenal surgery with extra corporeal shockwave lithotripsy for lower pole renal calculi

Abstract
Treatment of stone disease is based mainly on location of stone and stone burden. The current treatment options range from shock wave lithotripsy (SWL), Percutaneous nephrolithotripsy (PCNL) to Retrograde Intra-Renal Surgery (RIRS) based on patient and stone characteristics. Our study aims to compare the efficacy of RIRS with ESWL for lower pole renal calculi and to analyze the safety of RIRS in comparison with ESWL for lower pole renal calculi. Sixty patients were included in the study, out of which 30 patients underwent RIRS and 30 patients underwent ESWL for lower pole renal calculi of size<1.5cm. The success rate for ESWL is around 73% and that for RIRS is 93%. We kept the success size as 4mm for SWL and 3mm for flexi URS. Minor and major complications encountered were discussed. ESWL can be used as first line therapy for stone size<1.5cm. RIRS can be used for failed ESWL patients, morbidly obese patients and in case of anatomical abnormality of kidney.
