Factors affecting hospital length of stay among surgical patients in a tertiary care institution

Abstract

Background: Hospital length of stay (LOS) is an indicator of health service efficiency. Since hospitals lack detailed cost data, they commonly use length of stay as a proxy for resource utilization. The best strategy to reduce excessive LOS among surgical patients is unclear. The prolonged LOS could be due to postoperative complications or due to mode of surgical practice eg: minimally invasive surgery can reduce LOS. Reducing LOS can reduce hospital acquired infections, cost to the patient , readmissions and hospital can serve more patients Objectives: To assess the hospital length of stay among surgical patients admitted to general ward and to analyse the independent risk factors influencing LOS Results: There were totally 782 patients included in the study and were analysed, 519 (66.4%) patients were males and 263 (33.6%) patients were females .They were further divided according to the mode of payment and various categories of surgery they underwent. 573 (73.27%) patients belonged to corporate type(payment through various schemes) and 209 ( 26.7%) patients belong to cash type(payment through cash) . The LOS among patients belonging to corporate group (14.66 days) was significantly higher compared to an average of 9.53 days among cash group (p value <0.001) Conclusion: There are many factors which prolong LOS among surgical patients. In this study the mode of payment was analysed and found to be significantly affecting the LOS. .
