Perforator flaps for the reconstruction of achilles tendon region defects of lower limb

Abstract

Background: Management of Achilles tendon region defects is a formidable challenge for the reconstructive surgeons. Even small defects at this region can become problematic, due to thin non-expendable soft tissues and predisposition to massive oedema formation in the lower limb. The options for reconstructing such defects include local random flaps, reverse flow island flaps, and free flaps. Local flaps based on perforator vessels are raising interest in the reconstructive surgery of the limbs. The aim of this study was to evaluate the effectiveness of perforator flaps in the coverage of soft tissue defects in the Achilles tendon region of lower limb. Methods: In this prospective study, 12 patients, with Achilles tendon region defects of various aetiologies, and, treated with various flaps based on perforators arising from peroneal artery and posterior tibial artery were included in the study. A hand-held ultrasound doppler was used preoperatively and intraoperatively to detect the perforator vessels. Results: Out of the 12 patients, there was partial flap loss in 1case and distal flap necrosis in another. Minor complications included infection and wound dehiscence (2 cases). All wounds were eventually healed by the last postoperative visit. Conclusion: Perforator flaps may represent a reliable alternative for closure of small to moderate sized soft tissue defects of Achilles tendon region, with decreased donor site morbidity.
