DEPRESSION AND THE HEART

Abstract
Depression is common in patients with cardiac ailments & few studies have reported incidence of depressive features in upto 33% of individuals after a heart attack. The patho-physiological mechanisms seem to be multipronged & variations in sympatho-adrenal & hypothalamic-adrenocortical systems tend to playa major role. The psychiatric care of the depressed cardiac patient encompasses three principal modalities like psychotherapy, drug management & ECT. The drug management in this aspect has seen a shift of preference from TCA to SSRI over the past decades. This is primarily due to some recent trials demonstrating fewer cardiac side effects of this class of agents. Awareness about the possible cardiac side effects of other class of drugs & ECT is crucial in providing a wholesome therapeutic strategy to depressed cardiac patients.
