Emergence of the Pandrug Resistant Gram Negative Rods in Tertiary Care Hospital of Lahore, Pakistan

Abstract

Background: The incidence of multi-drug resistant Gram-negative bacteria have been dramatically increased in the current era. The dilemma of the Pandrug resistance is the most feared predicament at present and for the coming future. The objective of this study was to determine the distribution of the Pandrug resistant Gram-negative rods in various clinical samples and to determine the most common Gram-negative rods with pandrug resistance. Materials and Methods: All the clinical samples received for culture and sensitivity were processed using standard microbiological methods. Gram-negative bacteria were isolated and identified using bench tests and biochemical analysis. The antimicrobial sensitivity was performed by modified Kirby-Bauer disc diffusion method as described in CLSI-2017 guidelines. Results: A total of 14030 clinical samples were processed for the culture-sensitivity among which 6854 (48.85%) were Gram-negative rods. Among them 108 (1.6%) of the Gram-negative rods, had depicted the pandrug resistance. The most common pandrug resistant bacteria including, Klebsiella sp., Pseudomonas sp., Acinetobacter sp., and E. coli. The pandrug resistant bacteria were most commonly isolated from pus samples and wound samples followed by other samples. Conclusion: The isolation of pandrug resistant Gram-negative rods is an alarming situation for the healthcare system, demanding immediate action by the health authorities to deal with the menace. In this regard, coordinated interventions like antimicrobial stewardship program could be an effective way to improve the use of antibiotics.
