Evaluation of Cardiovascular Risk Factors in Psoriasis with Special Reference to Highly Sensitive C - Reactive Protein

Abstract

Background: Psoriasis is a chronic, multisystem, immune mediated, inflammatory disease characterized by epidermal hyper proliferation, abnormal keratinocyte differentiation, angiogenesis with blood vessel dilatation and Th1, Th17 inflammation associated with remission and relapses. Associations between psoriasis and metabolic diseases and increased mortality from cardiovascular diseases in patients with severe psoriasis have been recorded. Aims and objectives: The aims and objectives were to assess the cardiovascular risk factors in psoriatic patients, to know the importance of HsCRP, a general biomarker of inflammation, in psoriatic patients for cardiac risk and to estimate cardiovascular risk in psoriasis using the calibrated risk score. Materials and methods: All moderate to severe psoriatic patients attending outpatient department of a tertiary hospital in South India were included. General data along with history of CVD and cerebrovascular disease were collected. Thorough clinical examination was done to know the severity of disease and other co morbidities. Severity of psoriasis was calculated based on PASI score and BSA. Based on these findings, the cases were classified into mild, moderate and severe. Cases were screened for the presence of metabolic syndrome as defined by NCEP Adult Treatment Panel III 2001. Results: Among the total 100 patients of psoriasis, 71 were males and 29 were females with M: F of 2.3:1. The maximum number of patients seen were in the age group of 40–50 years of age (37 %). Prevalence of metabolic syndrome in psoriatic patients was 32 %. There was a statistically significant association (p value - 0.007) between age & metabolic syndrome. HSCRP correlation with waist circumference, PASI score, and Fasting Blood sugar was statistically significant. Conclusion: HsCRP and PASI could be used as a powerful and sensitive blood marker to evaluate psoriasis disease severity and can be used to monitor the disease course and treatment. Elevated levels of HsCRP may be an independent risk factor for cardiovascular disease in psoriatic patients. Elevated HsCRP and BMI are indicative of close relationship between mild/moderate inflammation and obesity related metabolic abnormality in psoriasis.
