Evaluation of concordance between the cytological grading and histomorphological grading of breast carcinoma in a series of 100 cases

Abstract

Background: Breast cancer survival is linked to early detection with timely appropriate treatment. Fine needle aspiration cytology is a reliable method for the initial evaluation and diagnosis of alpable masses of the breast. The grading of breast cancer on fine-needle aspiration shows almost comparable tumor grading as seen on the post-surgical histopathological specimens, hence giving us the leverage of early diagnosis with subsequent early beginning of treatment and a potential benefit in early starting of neoadjunct therapy in cytoreduction of breast carcinoma masses. Aims and Objectives: Comparative study between the cytological grading of breast carcinoma on fine needle aspiration smears using Robinsons grading method and the histological grading of these cases on histopathological sections using the Modified Bloom-Richardson-Elston grading system to calculate the concordance between them. Design- A Two year Longitudinal Prospective Study Materials and Methods: FNAC smears of 100 female breast carcinoma patients were stained with HandE stain and PAP stain and were graded using Robinsons Cytological Grading system. Subsequent postsurgical specimens of these patients were processed for histopathological evaluation and grading using the Modified Bloom-Richardson-Elston grading system. Concordance between the two grading systems was calculated. Results and Conclusion: The study showed concordance between the cytological and histological grading system to be 85.19% for grade I, 95% for grade II and 76.92% for grade III, with an overall concordance rate of 90% hence proving that cytological grading is helpful in predicting histological grade and biologic behaviour of tumor and therefore it should become a part of routine cytology reporting of carcinoma breast cases.
