Laparoscopy in surgery of inflammatory bowel disease

Abstract

Since the first reported successful series of laparoscopic colorectal surgeries twenty years ago, laparoscopy was willingly used by increasing number of surgeons in numbers of indications. It has been showed that laparoscopy for inflammatory bowel disease (IBD) yields comparable or, in some aspects, better results in comparison to open surgery. It concerns: quicker return of gastrointestinal functions, shorter postoperative hospital stay, faster recovery and, in particular, reduced rate of postoperative bowel obstruction and superior cosmesis. The two last elements seem to be particularly important in IBD patients – very young people and very often requiring more than one surgical procedure during their lifetime. However, some disadvantages of laparoscopy in inflammatory bowel disease still exist. Procedure requires longer operative time, expensive endo-stapling devices, advanced technical expertise and learning curve prior to improvement in outcome. The substantially demanding procedure of laparoscopic bowel surgery is even more challenging in IBD patient. In present article authors try to review current concepts and controversies in laparoscopic surgery of patients with inflammatory bowel disease.
