A study of spinal anaesthesia in paediatric age group

Abstract

 BACKGROUND: Spinal anesthesia has also been suggested for patient in whom General anesthesia may pose a significant risk as those with facial anomalies and difficult intubation, muscular dystrophy, family history of malignant hyperthermia or full stomach with aspiration risk. Study was done to study characteristics of spinal anaesthesia in paediatric patients. MATERIALS AND METHODS: 60 patients of 3-13years age, ASA I-II planned for infraumbilical surgeries were selected. All patients were sedated with syrup midazolam 0.5mg/kg 15 to 20 min before induction. In operation room all patients sedated with oxygen + nitrous oxide + sevoflurane. Spinal anaesthesia given with injection hyperbaric bupivacaine (0.5%) 0.4mg/kg. Maximum cephalad extent of block tested by lack of response to pinprick method at dermatome level. RESULTS: Mean height of sensory block was T6 (T4 to T10), time to two segment regression of block was 53.1±12 min; time to regression of block at T10 was 82.2±21min. There was marked hemodynamic stability in paediatric patients during spinal anaesthesia. Injection mephentermine was required in one patient to treat hypotension; three patients (5%) were given general anaesthesia due to prolong duration of surgery. There was difficulty in locating subarachnoid space in four patients(6.6%), One patient was given respinal due to inadequate sensory and motor block. Two patients (3.3%) developed nausea, vomiting and no patient developed post puncture headache. CONCLUSION: Paediatric spinal anaesthesia is safe, reliable and effective technique with marked hemodynamic stability in procedures of duration less than 75 to 120 minute.
