Role of Peripheral smear in early diagnosis of neonatal septicaemia: A three year study at P.D.U. Medical College, Rajkot

Abstract

BACKGROUND: A study of 120 neonates with clinical suspicion of septicemia, admitted in neonatal intensive care unit of K. T. Sheth Children Hospital, Rajkot was carried out to study the role of peripheral smear, to evaluate the usefulness of white blood cell parameters and to establish the sensitivity and specificity of each tests for early diagnosis of neonatal septicemia. MATERIALS AND METHODS: The prospective study of 120 neonates admitted in the neonatal care unit of K.T. Sheth Children Hospital, during the period of November 2010 to June 2012 and all tests were performed at the department of Pathology, P. D. U Medical College, Rajkot. Blood sample was obtained by peripheral venipuncture and the sepsis work up included blood culture and routine blood counts. The total leucocytes counts were counted on a utomated hematology analyzer. Leishman stained peripheral blood smears were used for the study. RESULTS : Based on clinical findings and laboratory data infants were classified into three categories: Sepsis (n=58), probable infection (n=42 ) and non infectio n (n=20 ). The commonest organism isolated were Staphylococcus aureus (31.03%) and Klebsiella Pneumonia (24.13%). Abnormal I:T Ratio (≥0.2) show highest sensitivity(84.48%) and specificity(95%). CONCLUSION : Peripheral smear study is found very useful in ear ly diagnosis neonatal septicemia as it can be performed even in small laboratory without any special equipment and within a short period of time. Among white blood cell parameters I:T Ratio show highest sensitivity and specificity.
