PERCUTANEOUS NEPHROLITHOTOMY IN THE TREATMENT OF STAGHORN RENAL LITHIASIS - PERSONAL EXPERIENCE

Abstract

Staghorn calculi were falsely believed to be well tolerated and, consequently, not needing surgical treatment. The therapeutic approach towards staghorn calculi is currently more aggressive due to the consequences on the parenchyma and the renal function. The analysis of personal experience in endoscopic treatment of staghorn renal lithiasis over a period of 26 years (1988 – 2014), where endoscopic treatment and extracorporeal lithotripsy have replaced almost completely classic surgery (open surgery). On a group of 7824 patients with renal lithiasis, of which 1174 (15%) had staghorn lithiasis, and 17/1174 (1.44%) patients had bilateral staghorn lithiasis, and 7/1174 (0.59%) were children.The average length of postoperative stays was 9.14 days. The average length of operative time was 94.5 minutes. The "stone free" rate at the end of the intervention was 972/1174 (82.75%). Postoperative complications were present in 164/1174 (13.96%) cases, with 5/1174 (0.42%) deaths, 2 cases of haemorrhage, 2/1174 cases of septic and toxic shock, 1/1174 (0.08%) due to a congenital cardiac abnormality - arrhythmogenic right ventricle dysplasia. For the successful endoscopic resolution of a staghorn calculi, it is necessary the proper conduct of the pathway of percutaneous nephrostomy (NP) by having a better knowledge of the three-dimensional anatomy of the pyelocaliceal system (SPC), best training, expertise and the best technique possible in the field of endourology of the upper urinary tract.
