Pleuropulmonary lesions of connective tissue diseases in a resource-limited country

Abstract
Introduction. In Africa, pleuropulmonary disorders of connective tissue diseases are under diagnosed and sometimes diagnosed late. Our study aims to describe the epidemiological, clinical and paraclinical aspects of pulmonary disorders of connective tissue diseases in subjects having the consultants from Yalgado Ouedraogo University Hospital (CHU-YO) in Ouagadougou to improve patient care. Methods. This is a descriptive and analytical study that took place over a six-month period from January 1st to June 30th, 2016. It involved patients with connective tissue diseases over the age of 15 who were admitted to the pulmonology, dermatology and rheumatology departments of CHU-YO . Results. Fifty-five cases of connective tissue diseases were collected including 23 cases of rheumatoid arthritis; 13 cases of systemic lupus erythematosus; 10 cases of systemic scleroderma; 6 cases of dermatomyositis/polymyositis; 4 cases of indeterminate connective tissue disease (or mixed syndrome or SHARP) and 1 case of Sjögren's syndrome. The sex ratio was 0.29. The respiratory manifestations were cough, dyspnea and chest pain. The radiological lesions were bilateral in 84.21% predominantly basal and interstitial. The lung functional abnormalities were represented by a restrictive disorder in 56.25%, a obstructive disorder in 31.25% and a mixed ventilatory disorder in 12.5%. Conclusion. The respiratory manifestations of connective tissue diseases are not specific but their management must be early to avoid the occurrence of respiratory complications leading to morbidity and mortality.
