Primary cutaneous anaplastic large T cell lymphoma: Very unusual presentation

Abstract
Background: The primary cutaneous anaplastic large cell lymphoma (C-ALCL) is a non-Hodgkin lymphoma (NHL) of cutaneous T-cell presentation, without systemic involvement. It is common in the sixth decade of life and in male. Solitary or localized cutaneous lesion is common over the trunk, face, extremities and buttocks However, multiple lesions are seen in 20% cases. 75% of Tumor cells express the CD30 antigen. Histologically. These lesions show large sized T lymphocytes with anaplastic cells. Aim: To study high grade anaplastic cutaneous lesions of lymphoid cells and other related differential diagnosis including adnexal tumors or metastasis. Result: Histopathological examination shows tumor cells having lymphoid differentiation with mitotic figures in papillary dermis. Tumors cells also infiltrate lower part of dermis with interstitial pattern. Immunohistochemistry-Tumors cells are atypical CD30 positive and CD8 focally positive. Conclusion: Primary cutaneous lymphoma are diagnosed by light microscopy and IHC. So in our case with the help of IHC, it is suggestive of Primary cutaneous anaplastic large T cell lymphoma. Compared to other T cell lymphoma this entity is very rare.
