Special review and analysis on the clinical unreasonable use of proton pump inhibitors for injection

Abstract

<b>Objective</b> To analyze the irrational use of proton pump inhibitors (PPI) for injection in clinical practice and its reasons, and to improve the rationality of PPI through appropriate intervention measures. <b>Methods</b> The consumption of PPI in June 2023 was retrieved from the hospital information system of Nanjing Medical University Affiliated Brain Hospital/Nanjing Chest Hospital. The irrational use of PPI during the use process was classified, counted, and summarized. <b>Results</b> A total of 789 PPI for injection were used in Nanjing Medical University Affiliated Brain Hospital/Nanjing Chest Hospital in June 2023, including 569 injections of esomeprazole, 84 injections of omeprazole, 42 injections of esomeprazole, 41 injections of lansoprazole, 36 injections of pantoprazole, and 17 injections of rabeprazole. Ninety-one cases of irrational medication orders were found, including over-treatment (36 cases, accounting for 39.56%), off-label use (23 cases, accounting for 25.27%), inappropriate dosing (23 cases, accounting for 25.27%), and inappropriate frequency (9 cases, accounting for 9.89%). <b>Conclusion</b> The phenomenon of irrational use of proton pump inhibitors in hospitals is serious, including empirical use, habitual use, and preventive use. Pharmacists should take various effective countermeasures to actively intervene, timely feedback the information of irrational use to clinical practice, promote the rational use of proton pump inhibitors in clinical practice, and improve the level of rational drug use in clinical practice.
