Two clinical cases of opsoclonus-myoclonus syndrome

Abstract

А detailed description and analysis of the detected cases of opsoclonus‑myoclonus (OMS) syndrome is provided. Two clinical cases have been analyzed in men who were under the hospital examination and treatment. In the first case, the onset of the disease was accompanied by severe dizziness, trembling gait while walking until it became impossible, tremor in the upper limbs when performing purposeful movements and periodic tremor in the circular muscles of the eyes. The onset was abrupt 2 weeks ago, on the eve of nothing sick, 5 months ago was vaccinated of measles. He pointed out that the disease was preceded by a stressful situation. In the second case, similar symptoms also appeared suddenly. However, the patient suffered from arterial hypertension, dyslipidemia and atrial fibrillation. In the neurological status of both patients, a combination of myoclonic hyperkinesis of the eyes and the trunk with a decreased tonе in the limbs, severe static and dynamic ataxia were observed. There were also psycho‑emotional and vegetative disorders. Patients underwent a comprehensive laboratory and instrumental examinations. The results did not show a clear relationship between the revealed symptoms with infectious factors or tumors, there were no vascular, degenerative or demyelinating lesions of the brain. The analysis of different therapeutic tactics were carried out. In both cases, positive effects of treatment tactics were noted, but the effectiveness of the treatment depended on the onset of its appointment. In the occurrence of OMS the key factor is the lesion of the cerebellum and its connections, mainly afferent. The absence of an exact pathogenetic mechanism of the syndrome occurrence causes the establishment of a syndromological diagnosis and a careful diagnostic tactics.
