A clinical study of upper gastrointestinal endoscopy in 1000 cases

Abstract

Background and objectives: Before the advent of endoscopy direct access to the lesion for the confirmationof the diagnosis was difficult, this posed difficulty in contemplating adequate andappropriate surgery. Endoscopy as a medical diagnostic tool has grown like a giant inrecent years. Upper Gastrointestinal endoscopy is one of the most fascinating branchwhich serves not only as a means of resolving or amplifying the diagnosis madeclinically or by x-ray ,but also a primary diagnostic procedure for conditions nototherwise diagnosable on un operated case.Fibre optic upper GI endoscopy has already become firmly established as areliable, quick and inexpensive tool. This study was done to know the efficiency ofendoscopy in the diagnosis of upper GI lesions, its impact in GI surgery. Its value inprognosis of upper GI disorders and its usefulness in follow up clinics were studied.The therapeutic applications of upper GI endoscopy and its futures prospects wereanalysed.Methods: All patients reporting to outpatient department and also the inpatients inwards of General Surgery and other Departments, who have upper GI symptoms andwere advised endoscopy at Basaveshwar Teaching and General Hospital attached toMahadevappaRampure Medical College, Gulbarga from a period of august 2015 to august 2018, were taken for this study.Results: Of the 1000 cases, 604 were males and 396 were females. Disease incidencewas highest in 21-30years age group i.e. 21.6%.Pain abdomen was the most commonsymptom.. Epigastric tenderness was the most common sign among the patientsclinically. Reflex esophagitis and erosive gastritis formed most common cases(319).Incidence of duodenitis - 5.8%, peptic ulcer-3.4%, oesophageal varices-4.8%,carcinoma oesophagus-3.9%, carcinoma stomach-0.7%, normal study-17.4%Conclusions: Upper GI lesions were more common in males. Incidence of diseases washighest among young adults, most of them being benign mucosal lesions mainly dueto spicy food and tobacco habits. Incidence of malignancy was mostly among olderage group above 40 years. Incidence of normal study was high owing to increasedmedical care, easy availability of the procedure and increased medical awarenessamong patients. In all these cases upper GI not only helped in diagnosing the diseasebut also helped to get information about pathology, extent of disease andcomplications that have occurred. The study highlighted the high diagnostic and therapeutic value of upper GI endoscopy. The study proved its safety, reliability, and the lowest morbidity and mortality associated with it.
