An unusual case: Primary synovial sarcoma of kidney.

Abstract
Primary synovial sarcoma usually occurs in extremities of young adult, only less than 50 cases are reported in kidney. It accounts 1% of adult maignant renal tumour. It is also known as embryonal sarcoma of kidney. Here we present a case of 48 yr male presented with abdominal lump. CT scan revealed mass in Lt.kidney in middle & lower pole. Lt.radical nephrectomy was done. On histopathology mass reported as malignant spindle cell sarcoma. Immunohistochemistry (IHC) was done and tumour cells were positive for Cytokeratin, EMA, Mic-2, bcl2, Calponin (Focal) & TLE-1. Nuclear INI-1 was mosaic in nature. This tumour represents a diagnostic dilemma because it is quite difficult to differentiate it from other renal neoplasm, such as primary & metastatic sarcoma, renal cell carcinoma with sarcomatoid differentiation, which may have similar histological features. Here we present a case report with brief review of literature.
