Comparison of concentration technique for detection of coccidian parasites among HIV seropositive patients and its correlation with CD4 T cell count

Abstract

Aim and Objectives: To study the prevalence of coccidian parasites among HIV seropositive patients and to compare concentration techniques for detection of these parasites and also to evaluate the association between CD4 T cell count and presence of coccidian parasites. Material and Method: Stool specimens were collected in wide mouthed, clean, dry, plastic containers with tight fittings lids from 510 patients who were HIV seropositive with or without diarrhoea. Microscopic examination was done by direct saline and iodine mount. All the specimens were subjected to concentration techniques for detection of coccidian parasites. About 5 ml of venous blood was collected from all patients in a vacutainer containing K2 EDTA for estimation of CD4/CD8 count. Result and Observations: Out of 510 patients, 161 (31.57%) had CD4 T cell count less than 200 cell/ µl. The maximum incidence of diarrhoea was seen among patients with CD4 T cell count less than 200 cells/ µl. A total of 67 (13.33 %) coccidian parasites were detected by formol ether sedimentation technique, While 78 (15.29%) were detected by Sheather’s sucrose floatation technique. Detection rate of intestinal parasite was more in patients presented with diarrhoea having CD4 T cell count less than 200 cells/µl. Conclusion: Detection rate of coccidian parasite was more by concentration technique as compared to direct saline mount and detection of coccidian parasites by formol ether centrifugal sedimentation technique followed by modified Z-N staining is as effective as Sheather’s sucrose floatation technique. CD4 T cell count has been an excellent indicator which defines the degree of immune suppression.
