Diagnosis and treatment of Amanita phalloides poisoning

Abstract

Mushroom poisoning continues to be a serious clinical problem in Poland. Although it has relatively low incidence, neglecting this etiology in differential diagnosis in gastrointestinal disorders may postpone initiation of appropriate treatment with potential fatal outcomes. The most serious are intoxications with mushrooms containing cytotropic toxins with predominant injury of liver, kidneys and heart. Many cytotropic toxins are amatoxins, gyromitrine, and orelanine. The Amantia phalloides intoxication is the most important due to its high mortality reaching 12-50%. Thus, in suspected Amantia phalloides intoxication appropriate treatment should be instituted immediately, even before laboratory diagnosis confirmation.The current data concerning epidemiology, clinical picture, diagnostics and treatment in Amantia phalloides poisoning is presented. Despite progress in therapy this poisoning caries serious morbidity and high mortality.
