Discard of blood & blood components- How much is acceptable ?

Abstract

Objective and Background -.Human blood is an essential element of human life and there are no substitutes. Blood is a valuable resource and blood wastage could impose a very serious impact on healthcare. The purpose of this study was to determine rate and causes of wastage of blood and its products. Material and Method - A retrospective study was conducted in Department of Immuno-Haematology & Blood Transfusion (IHBT), Civil hospital, Ahmedabad. It involves various reasons of wastage of whole blood and blood components. Data was retrieved from Blood Bank Data Management Software System (BDMS). Result - Total 96421 components were prepared in study period, including red Cells, platelet concentrates, fresh frozen plasma, cryo precipitates and cryo poor plasma. Total 3132 components were discarded during study period. Discard rate was 3.24%. Out of 3132 discarded blood and its product, 971 (31%) were discarded due to seropositivity. Highest discard rate amongst component was FFP (5.43%). Conclusion - Number of reasons are responsible forBlood wastage , including transfusion transmitted infections (TTI) seropositivity, leakage, lipaemia, RBCs contamination, suboptimal volume, time expiry, blood medically or surgically ordered but not used, hemolysis or miscellaneous reasons. The blood transfusion system has made significant advancements in areas such as donor management, storage of blood, cross-matching, rational use of blood and distribution. In order to improve the standards of blood banks and the blood transfusion services, comprehensive standards have been formulated to ensure better quality control in collection, storage, testing and distribution of blood and its components for the identified major factors affecting blood product wastage.
